During your Bloodborne run, there is only one spot where you will certainly find a Bloodstone (the others will have random locations). The place in question is located inside the Mensis Nightmare, precisely at the attic of Mergo.
First exit the room and go up the stairs to the left. There will be an area where it will be possible to fall, near the corner to your right: be careful not to go all the way down or you will leave your feathers there. About mid-fall you should find an opening to slip into. If you fail on the first try you will find the Blood Echoes right where you dropped off. Once you land on the right level, go to the end of the hall and then turn left, where you will notice 3 enemies, the Winter Lanterns. Such creatures are perhaps the most dangerous in Bloodborne: do not fight them, avoid their attacks and overcome them.
The first Lantern can be easily avoided: wait for it to turn to pass it. To go unnoticed by the next two you will have to wait on the left, until the closest one goes towards the right of the bridge: immediately run towards the building in front of you. You will probably be hit and fall victim to insanity but treat yourself with a sedative and you can reach the room immediately ahead. However, stay away from the door to avoid constant attacks.
There will be four spiders inside the room, just avoid them and keep going to the rooms ahead. Get to the end of the path to find a lever: pulling it will bring down another lantern that otherwise would create problems in taking the stone. Then go back to the previous room and throw yourself into the opening you will find.
Go back to Mergo's Attic and cross the bridge where you fought Micolash, Host of the Nightmare. Go down the stairs to the left for 2 flights of stairs and then keep going straight, down the circular stairs. Then go left and, when you meet the fog, keep going down two more ramps, then turn right. Look at the wall on your left for a corridor that would otherwise be difficult to see. At the end of the corridor there will be a gate, to open it you will need the Iron Door Key.
After opening it continue to follow the path, going up the stairs. If you have not pulled the lever we were talking about before you will find yourself in front of a Winter Lantern, which will create considerable problems for you. The Bloodstone is located right in that room.
